Fines: $1,500–$6,250

Test Refusal Penalty

Same penalties as failing a DUI test — 1-year suspension.

Getting Your License Back

Complete DMV Form 735-6044, pay $50 application fee + $75 reinstatement fee. Judge recommendation required (except Implied Consent). SR-22 insurance and approved IID required.
